- How many visits are covered per year?
- Are acupuncture benefits combined with chiropractic and/or massage therapy services?

- Is there a separate co-pay or co-insurance for a new patient visit and evaluation (CPT code 99203)?
- Is pre-authorization required before starting treatment?
